The purpose of this data capture is to ensure accurate recipient configuration of your department’s fax line. Upon form submission, we will start the process of migrating your fax line to the Los Rios Fax Server. Click here to see the Fax Server Getting Started Guide.

Please send all Fax Server related requests via the KACE ticketing system and under Category, select FAX in order for your ticket to be routed to the correct department. Generally, your campus Operations department would handle this request on your behalf.
